Members save $25.00 on these in- depth telephone assessments, which result in a customized, comprehensive, written care plan. The care plan may include a review of the current living situation, recommendation for alternative care settings, a safety and risk assessment, and recommendation for types of caregiver services. Care coordination may include assessment of informal support provided by caregivers, coordination of community services, and arranging placement in an elder care facility.
